I hereby acknowledge and agree that my rental of equipment from Cellhire shall be subject to a Rental Agreement of even date herewith, containing Cellhire’s standard terms and conditions, which are hereby incorporated herein by reference. I hereby further authorize Cellhire to bill all wireless rentals and any other relevant charges to my current credit card as detailed on this application form. If approved, I understand that any charges billed to my Amex®, Mastercard®, or Visa® account will be subject to other charges that appear on the account in accordance with the terms and conditions governing that account. Each time I rent a wireless phone it will be subject to authorization before my reservation is confirmed and equipment dispatched. Should I cancel my order, I agree to pay any incurred costs and cancellation charges. Should my card details change at any time or my card is lost or stolen, I agree to notify Cellhire and the appropriate credit card company immediately. If I wish to terminate this authorization to bill my account, I will notify Cellhire and the relevant credit card company.
